Has anyone had any type of surgery to treat their TMJ? My dentist is referring me to an oral surgeon for a type of surgery that is supposed to clean out from around the jaw joints. Has anyone else had this procedure done and did it work well for you?

The only girl I know who even had something similar ended up having an ER surgery which is notling like (I think) your dentist is recommending. She had ended up waiting until it had gotten so bad that the dentist had to remove part of her jaw. :(

I read a little about this and most sites say that this comes from many types of medical problems. Is there any way to narrow down what actually causing is and it the surgery always needed as an end result? I would assume its a last measure type.

I have had the surgery that you are refering to and it's called arthrocentesis. It is similar to a surgery they can do around your knee. Basically they make a small incision insert a tiny camera and then a little miniature hose to flush out debris from around the joint. In my experience it helped for a short while but in no way was a long term solution.

MJ is often treated using uncomfortable orthodontic devices. The goal is to reposition the lower jaw using appliances that decompress the joint. If this procedure is not successful, and it can take a year or more to determine if it will be successful, the next step is surgery. My uncle had this surgery, found it very risky and not completely sucessful.
